You can provide your information and health history before entering our office via
MyPatientVisit. After receiving your appointment confirmation email, login, and follow the prompts carefully.

NOTE: If you are unsure of where your forms are, use the MyChart menu item to navigate to Patient Information and Forms.

You can also print the following documents, fill them out, and bring them to your appointment.

On the MyPatientVisit login page, click the “Need help with your login?” link and follow the prompts.

I don’t have an account – What do I do?

If you have an appointment scheduled with us, and your account registration has been initiated, please confirm your account with the information provided in your consultation confirmation email. If you are still unable to register, please call our office at 425.827.7878.

Where can I find my health questionnaire, skin profile, and/or demographics form?

On the MyPatientVisit dashboard, click Complete Registration or click myChart in the menu and navigate to Patient Information (demographics) or Forms (questionnaire/skin profile).

What is your Practice ID?

You can find our practice ID in your consultation confirmation email, along with your security code and information entered to initiate your account registration.

I can’t upload my ID – What do I do?

If you upload your ID, but return to the ID upload window (step 1) window later, it will not show the files you uploaded. To check if your ID has been uploaded – Click MyChart, then Document Summary to view all of the items you have uploaded. You may also skip this step and we can scan your ID at the time of your appointment.
