Bariatric Surgery (surgery for weight loss) has become an acceptable and viable treatment for morbid obesity. The consequence of weight loss after such surgery is dramatic in its positive effects on the health and well being of individuals who have suffered from the ravages of morbid obesity. However, this dramatic amount of weight loss can leave the body with an astonishing amount of excess skin, the loss of physiologic contour and architecture, and overall aging of all body parts. Weight loss body contouring can dramatically remodel a body that has undergone significant weight loss.
Sepehr Egrari MD FACS has a large national and international following for weight loss body contouring. His unique approach and innovative techniques have made him one of the most prominent mega body contouring surgeons in the country. He is involved with multiple clinical trials and spends a considerable amount of time presenting and instructing other surgeons about the most innovative techniques of mega body contouring. He has developed unique techniques for lower body lifting. He feels that every one's weight loss will manifest itself in a unique way, and therefore, the operation needs to be individualized to match the unique body structure.
Who is a candidate?
- Individuals with history of morbid obesity who have lost significant weight either by bariatric surgery or naturally

Procedures
- The general philosophy of all the procedures is to remove skin and to enhance the contour and architecture of the body by creating a lift of the affected parts
- A major objective of these procedures is to create a waistline, more attractive silhouette, and tighter appearing skin
Breast enhancement
- This could be a reduction, lift, or augmentation. The type and extent of the operation depends on the age and presentation of the patient.
- Many different innovative lifts and reductions are used to minimize scarring and maximize the enhancement and rejuvenation of the breasts. They include periareolar, vertical and standard lifts. Implants are occasionally used to enhance the results. One of Dr. Egrari's specialties is the vertical lift combined with placement of an implant. This is a short scar lift that provides long lasting and beautiful enhancement of the breasts.
Lower body contouring and lift
- Contouring the abdomen usually requires an Abdominoplasty (tummy tuck). Commonly it is known as a panniculectomy, however, Dr. Egrari believes that no one should have 'just a panniculectomy' - which simply excises the lower abdominal skin without any attention to the abdominal wall, flank contouring, or umbilicus.
- One of the main objectives of the lower body contouring is the process of recreating the waistline. Dr. Egrari calls these "waist building" operations.
- There are four general types of Abdominoplasty:
- Abdominoplasty
This is the simplest form, which includes excision of the lower abdominal wall skin, along with plication (tightening) of the abdominal wall. Minimal amounts of liposuction are done on the sides to create a smooth contour. The incision is from hip to hip and around the umbilicus. This is for individuals who have mild excess skin of the lower abdomen.
- Extensive Abdominoplasty
This is an extensive operation, which includes excision of massive excess skin of the lower abdomen. The abdominal wall is plicated and the flanks are moderately liposuctioned. Most of the time this type of Abdominoplasty requires a "fleur de lys" excision, which includes a vertical excision of upper abdominal wall skin. This allows enhancement of the flanks and is the most common type of Abdominoplasty. The scar is an inverted T type incision.
- Abdominoplasty with Flank Extension
The extension into the flank is ideal for individuals who have a redundancy of skin and fat past the hip area into the lower back. This is an effective Abdominoplasty and can dramatically recontour the entire lower trunk. Significant plication of the abdominal wall and occasionally sides, along with dramatic liposuction, makes this one of the definitive tools in lower abdominal recontouring.
- Lower body lift (Extensive belt lipectomy)
This is the gold standard of body contouring and one of Dr. Egrari's specialties. It is a dramatic recontouring of the entire lower trunk and abdomen. It removes excess skin and also creates an effective lower body lift of the thighs, buttocks, and the pubic area. The scar is a circumferential one that is designed to be hidden. This is another important operation in regaining the silhouette that is so desired. The lower body lift is truly a versatile operation that at its core pays attention to "waist building".
Brachioplasty
- This is a procedure that is used to excise the excess skin of the upper arms.
- Ideally it is combined with breast procedures, which allows an innovative connection of the two areas to reduce redundancy of excess skin of the outside upper chest wall and axilla (arm pit). This is another unique operation that Dr. Egrari specializes in. It is the only operation that is effective in removing the bothersome skin excess that comes out of side of the bras that women wear.
- The scar runs from the axilla to the elbow. When an extensive brachioplasty is done a zigzag type scar runs on the side of the chest and removes the excess skin of the side area.
- Occasionally a limited one is done that places the scar only in the axilla.
Thigh-Lifts
- Medial Thigh-Lift
This is a procedure that is done to remove the excess skin between the upper thighs. It always involves significant liposuctioning. The incision is in the groin crease and follows the pubic hairline. A medial thigh lift is not done with the lower body lift and needs to be staged in a future operation
- Lateral Thigh-Lift
This procedure addresses the outer thigh excess skin along with the ptosis (droopiness) of the buttocks. An Abdominoplasty is frequently a part of this operation, as well as significant liposuctioning.

- During your pre operative visit you will undergo a detailed evaluation and will receive very detailed information regarding your surgical experience.

Recuperation and Healing
- Procedures can take anywhere from 2 hours (Brachioplasty) to 9 hours (Abdominoplasty, thigh lifts, breast rejuvenation).
- Depending on complexity, Dr. Egrari will determine if a patient will need overnight care with one of our specialized nursing services.
- All Abdominoplasty patients are recommended to get the On-Q pain pump, a percutaneous temporary system that significantly reduces pain in the first 3 to 4 days.
- Most patients will have post-operation garments.
- All discomfort is controlled with a variety of pain medications and muscle relaxants.
- Sutures are removed in 7 to 10 days.
- Drains are removed in 10 days. (Not all procedures need drains.)
- Most swelling and bruising is gone in 2 weeks.
- Final contour and definition is evident in 8 to 10 weeks.
- Most patients take 2 to 3 weeks off from work.
- Most patients resume all exercise regimens in 4 to 6 weeks.
Combinations
Dr. Egrari believes that most areas of the body are functional
and aesthetic units. For this reason, the following combinations
are recommended:
