We consider it a privilege that you have chosen us for your needs. We strongly believe that an informed patient is a good patient. We strive to inform you of all the medical aspects of your needs and also would like to advise you on our financial policy.
- The consultation fee is $75.00 and is due on the date of service. This fee will be credited towards a selected procedure, if the surgery is scheduled within one year of the consultation.
- Scheduling a case will require a totally nonrefundable
amount of $1000. This will be credited towards your surgery,
if the surgery is scheduled within six months of the consultation.
- Payment for surgery is due at the pre operative
examination or 2 weeks prior to the operation, whichever
comes first, and is non-refundable. If fees are not
rendered at that time a hold will be placed on the operation.
If fees are not provided 5 business days before the
operation the surgery will be cancelled.
- We consider the preoperative examination as a definitive
final consultation prior to the operation. The fee for
surgery that is rendered at the preoperative appointment
is non refundable and non transferable to another
segment of time. If the surgery is rescheduled
after the payment has been made there will be a 35%
rescheduling fee for the total cost of the surgery.
This is done to maintain the continuity of a very valuable
and busy surgical schedule. The only exception to this
is life threatening emergencies or medical illnesses
prior to the operation. In these exceptional circumstances
a 35% rescheduling fee will not apply. We reserve a
considerable amount of discretion in implementing this
difficult but necessary policy.
- It is the patient's responsibility to complete all
pre-surgical diagnostics prior to the pre-operative
appointment. If not completed by this appointment, surgery
may be postponed and subject to a 35% rescheduling fee,
or cancelled without a refund.
- Every effort is made to maintain the predicted operating
room and anesthesia hours. However results are NEVER
compromised for time. In these rare occasions that the
surgery outruns the determined time, you will be billed
for additional operating room and anesthesia fees.
- Plastic surgery is an art and occasionally revisions
will be necessary. These will always be within one year
of the operating date. The majority of the time no surgeon
fees will be charged, however facility and anesthesia
fees will apply for the procedure. Dr. Egrari reserves
the right to determine a revision versus a separate
procedure that is being requested.
- All returned checks will have a $35 service charge.
- There is a 3% surcharge for all credit card payments greater than $1000.
- We accept cash, cashiers check, personal check, and
the following major credit cards
- Visa and MasterCard
- American Express
- Discover card
- Anesthesia accepts Visa, Master Card, cashiers check
or personal check and is also due the day of your pre
operative appointment.
- We do not have in office payment plans.
- All financing is done through Cosmetic Fee Plan® and CareCredit®.
All arrangements have to be made prior to the preoperative
visit.
- As we spend a considerable amount of time in describing
the elective nature of these procedures, and as we fully
inform you of all the limitations and risks of the procedure,
we do not provide refunds for any reason.
- We do not accept any insurance as we are not contracted with any carrier. All procedures are subject to our appropriate fees. These include reconstructive cases. Some patients decide to bill their own insurance after the procedure.
